Benefits will be effective: December 1, 2018 through November 30, 2019
We are pleased to announce that we are adding some new plan enhancements that will be effective on December 1, 2018:
Anthem Blue Cross Dental
- New annual maximum carryover feature added. Earn money for visiting the dentist; your unused rewards will carry over each year.
- Composite fillings will now include posterior and front teeth.
Anthem Blue Cross Vision
- Elective contact lens allowance increased to $150 (from $130).
- Add, change or remove your dependent(s) in any of the following benefit options:
- medical, dental & vision
- medical & vision only
- dental only
